| Please read the following instructions carefully!
1. Full Conference registration fee includes: Monday Special Promotion Tutorials, Tuesday and Wednesday technical sessions, coffee breaks, lunch and a copy of the conference proceedings. To reserve your tutorial(s) seat you must select one or more tutorials you wish to attend! 2. Students receive a discounted registration fee with a valid student ID. Student registration includes: Monday Special Promotion Tutorials, Tuesday and Wednesday technical sessions, coffee breaks, lunch and a copy of the conference proceedings. To reserve your tutorial(s) seat you must select one or more tutorials you wish to attend! 3. One-day only registration entitles you to attend the technical sessions on your selected day, coffee breaks, lunch and a copy of the conference proceedings. 4. Exhibit-Only registration entitles you to attend the exhibition on both Tuesday and Wednesday along with access to the Tuesday afternoon panel. Exhibit-Only attendees can purchase a special promotion tutorial for an additional $50.00 or attend the regularly priced tutorials for an additional $200.00 per tutorial. 5. Full payment must accompany registration and may be charged to Visa, MasterCard, American Express, or paid by check made out to Design and Verification Conference. Telephone registrations will NOT be accepted. Note: For check payments from non-US banks, the registrant will be charged a collection fee of $30.00. Any registration forms sent without payment will be discarded. 6. All advance registrations must be postmarked no later then February 17, 2004. 7. REFUND POLICY: No refunds will be made unless written request for cancellation is received prior to February 17, 2004. All refunds are subject to a $25.00 processing fee. Requests for refunds received after the February 17, 2004, postmark date will NOT be honored and all registration fees will be forfeited. 8. There are a limited number of seats for each tutorial and it is common for them to sell out during advance registration. Registration for tutorials prior to the tutorial day is available only to conference registrants. There will be NO telephone reservations for tutorials. Payment must be received before you can be assured of a space. 9. Please indicate if you require a vegetarian meal. REGISTRATION DESK Advance registrants may pick up the Conference materials and others may register during the Conference at the registration desk located in the Bayshore Foyer.
